The film opens with Boyka in a brutal Russian prison. His knee was destroyed by George "Iceman" Chambers in the previous film. The prison bosses no longer care about him. He works a demeaning job and is mocked by other inmates. He cannot fight, and his pride is gone. He prays to God, promising that if he can walk again, he will change his ways.
A prison doctor says Boyka’s knee needs surgery or he’ll be crippled forever. A kind inmate (Albanian character named Viktor ) helps Boyka get a hidden stash of money. Boyka gets the surgery illegally in the prison infirmary. After the surgery, he makes a solemn promise to God : "I will be a good man. I will fight for something greater than myself."

Boyka, badly beaten, walks away from the prison. Jericho offers to help him, but Boyka says: "I fight for me. Go. You have a family." As he limps into the night, he smiles – not because he won the tournament, but because he finally kept his promise to God: he became a good man.
But Boyka cannot live with himself. He cheated. He lied. He won with deception, not honor. So he does the truly redemptive act:
